# Log Service Request (SR)

You can log Service Requests (SRs) related to various requirements, such as laptop requests, RAM upgrade request, keyboard request, software installation request, or information about leave policy, and so on.

To log a SR, perform the following steps:

2. Select the Department from the list.

5. Provide remarks and then tap **Next**. If no Approver is configured for the selected **Service Catalog**, the **Next** is not displayed, and the **Submit** is enabled. > The fields displayed above are based on the selected Service Catalog. The Administrators can configure the fields in the Service Catalog on the Form Builder page of the Web Application.

**Field Description**

The following table describes the fields on the SR Details page:

| Field | Description |
| --- | --- |
| Category | Displays the name of the Service Category to which the selected Service Catalog belongs. |
| Catalog Name | Displays the name of the selected Service Catalog. |
| Catalog Description | Displays the description of the selected Service Catalog. |
| Remarks | Specify the remarks for the SR. |
| Number of Approvals | Displays the number of approval levels configured for the selected Service Catalog. The level‐wise Approver names are displayed accordingly. > [!NOTE] > Note > > The newly logged SRs can be approved in parallel by approvers configured at the same level of approval. |
