Form Relations allows you to link two Transaction Forms in which one form would be the source form and the other target form. It also allows you to map fields and values in the source form with those in the target form. The fields and values from a source form can be successfully carried over to the corresponding fields and values of a target form.

Solution
In ACME organization, the Application Designer Sam creates an interface where the consumer can easily convert a logged incident to an SR and the details such as Tenant, Workgroup, Urgency, Priority, Symptom and Description of the incidents get mapped to the converted SR.

To achieve this functionality, the Application Designer Sam can create a Main Form named Manage Service Request. On Map Fields tab under (Form Designer > Form Relations) select the Source as Manage Incidents (with fields such as Tenant, Workgroup, Urgency, Priority, Symptom and Description). Select the Target as Manage Service Request. The fields available on Manage Incidents form gets automatically mapped to Target form Manage Service Requests.
